If you are a refugee and need to complete your Immigration Medical Exam, please follow the steps below:

  1. Fill Out the Form

    • Complete all required fields in the form below.
    • Ensure your contact details are accurate so we can reach you.

  2. Upload Your Refugee Document

    • Please upload a clear copy of your refugee document, which includes your unique reference number.
    • This document is required to verify your status before booking your appointment.

  3. Appointment Scheduling

    • Once you submit the form, our clinic will review your information and contact you to schedule your appointment.
    • Please note, if you choose not to wait and book directly online, you will be charged $100. This fee is non-refundable.
